HEALTHCARE JOURNAL OF ARKANSAS I NOV / DEC 2022 41 CLIENT ELIGIBILITY INFORMATION A woman is eligible for Maternal Life360 intensive care coordination supports if she: 1. Is enrolled in Arkansas Medicaid or was enrolled inArkansas Medicaid when she began receiving Maternal Life360 ser- vices and is either pregnant with a high- risk pregnancy (a diagnosis of needing supervision for high-risk pregnancy). 2. Or, was enrolled in the Maternal Life360 while pregnant with a high-risk preg- nancy and delivered the baby within the previous 24 months if enrolled in ARHOME at any point during enroll- ment in the Maternal Life360 program or within 12 months if enrolled only in another Medicaid aid category during Maternal Life360 enrollment. High-risk pregnancy must be verified through a completed referral form from the client’s physician that includes the most current clinical note. 3. Is not currently receiving state-funded or federally funded home visiting ser- vices through a provider whose services cover pregnancy or the first two years of a baby’s life. All adults living in the Rural Life360 ser- vice area are eligible for SDOH screening and referrals to needed community sup- ports. To be eligible for intensive care co- ordination, the individual also must: • Be enrolled inARHOME through a qual- ified health plan (QHP) or Medicaid Fee- For-Service (FFS). • Have a mental health and/or substance use disorder diagnosis. Nell Smith Assistant Director Division of Medical Services • Have at least one SDOH need identified through an SDOH screen. • Not be enrolled in the Provider-led Ar- kansas Shared Services Entity (PASSE) program. An individual is eligible for Success Life360 intensive care coordination and supports if the person: • Is enrolled in ARHOME through a QHP or Medicaid Fee-For-Service (FFS). • Is at risk of poor health due to poverty. • Is not enrolled in the Provider-led Arkansas Shared Services Entity (PASSE) program. • Meets the criteria for at least one of the following categories: • Is between 19 and 24 years of age and has been previously placed under the supervision of the DHS Division of Youth Services as verified by DHS • Is between 19 and 24 years of age and has been previously placed under the supervision of theArkansas Department of Corrections, as verified by the Arkan- sas Department of Corrections. • Is between 19 and 27 years of age and has been previously placed under the supervision of the DHS Division of Chil- dren and Family Services (as verified by DHS). • Is between 19 and 30 years of age and is a veteran verified by Arkansas Depart- ment of Veterans Affairs. HOSPITAL ELIGIBILITY INFORMATION To be eligible to enroll as a Life360 HOME provider withArkansas Medicaid, the entity must be an enrolledArkansas Medicaid hos- pital provider. Maternal Life360 HOME must be: • Licensed as a general hospital and pro- vide obstetrics services. • Able to demonstrate that it can provide or contract with a qualified partner or- ganization for the provision of a feder- ally recognized evidence-based home visitation model to a woman during pregnancy and to the woman and child for a period of up to 24 months after birth. Success Life 360 HOME must be: • An acute care hospital. • Able to demonstrate that it can admin- ister or contract with a qualified partner organization to administer programs using proven models, to provide or help connect individuals to employment, training, education, or other social sup- ports. Rural Life 360 HOME must be: • A small rural hospital. • Able to demonstrate that it can employ care coordination coaches to assist eli- gible individuals in need of treatment for serious mental illness or substance use disorder in accessing medical treat- ment from healthcare professionals and supports to meet health-related social needs. • Operate an acute crisis unit. n Nell Smith has two master’s degrees — one in journalism and the other in public health.
